Comparing the Costs of Obesity Medications
How much do obesity medications like Wegovy or Zepbound cost?" The answer is, that depends on the dose. So here is a breakdown of the current ways (as of Nov 4, 2025) that these medications can be dosed, and the costs per month related to those dosages.
Both companies provide lower dose options of their products that are not included in this summary. I include only the highest dose options because patients typically choose the highest potency medication because they provide the lowest price per unit of the medication. Patients are choosing to take these medications "off label" as a route to reducing their monthly cost.
About Counting Clicks
Each product has a set number of clicks that it takes to "dial in" their weekly dose of the medication. The estimated number of clicks required to reach a full dose of each medication is as follows:
Ozempic 1 mg. - 75 Clicks
Wegovy 2.4 mg - 75 Clicks
Moujaro & Zepbound 15 mg - 60 clicks
Rather than taking the recommended full dose that the pen is designed for, patients choose to dose a smaller amount by counting the clicks that pen makes in reaching the dose, this is a form of “off label” dosing. Manufacturers do not recommend it, but the practice has become quite common largely because the cost of obesity medication is high, and this practice reduces costs for patients.
If you are concerned that counting clicks is not an accurate way to dose your medication, you can simply count the clicks required to reach maximum dose on your own pen once you have it in hand to verify the estimate.

How long can I keep using these medications before they "expire"
This is a common question when considering off label dosing. Usually what is being asked is, "How long can I use Wegovy or Zepbound after I have started using it?”
Manufacturers report that they are not confident in the potency of these medications beyond the 2 month mark. This fact has not deterred patients or clinicians from usage of the products past those dates because doing so reduces costs significantly. At the time of writing this, I am unaware of any reports of harm coming from this practice.
